    • Health Form: log in to PowerSchool following the directions below to complete our Annual Health Authorization Form. If you prefer a paper copy, please let your school nurse know. Here are the step-by-step instructions:

          1. Log in to your own PowerSchool Account

          2. Click on the Forms icon on the left side
          3. Complete the following forms annually:
            • Health Authorization Form - Needs to be submitted each year for our Health Services staff to administer over-the-counter medications to your student during the school day.
            • Health Information - If you need to submit immunization updates or physicals, they can be uploaded here as well.
