Health Form: log in to PowerSchool following the directions below to complete our Annual Health Authorization Form. If you prefer a paper copy, please let your school nurse know. Here are the step-by-step instructions:

Log in to your own PowerSchool Account
- Click on the Forms icon on the left side
- Complete the following forms annually:

- Health Authorization Form - Needs to be submitted each year for our Health Services staff to administer over-the-counter medications to your student during the school day.

- Health Information - If you need to submit immunization updates or physicals, they can be uploaded here as well.

PRESCHOOL AND DISCOVER CLUB FORMS
Preschool and the Discover Club (before and aftercare) programs are licensed programs. This means all children in the facility are required to complete additional state forms to have on file upon request by the state to review yearly. Here are some of the forms.
